The NSW Outback Division of General Practice (ODGP) Keeping Healthy Puura Manti Program (KHPMP) services are available to adults 15 years and over with 2 or more chronic diseases or at high risk of due to pre-conditions or intergenerational connection. Services are aligned with and delivered in general practices and ACCHO’s in the identified high need ("hotspot")locations within North Western NSW (Lot 2).
Disciplines supported under KHPMP service delivery include Dietetics, Podiatry, Practice Nursing and Care Navigation, and encounters or occasions of service occur via face to face consults at clinics in general practices and ACCHOs and also complemented by Telehealth.
Allied health referral criteria of an accompanying GP Chronic Condition Management Plan (GPCCMP) or Health Assessment to verify 2 or more chronic conditions or at high risk of due to pre-conditions and intergenerational connection ensures that the general practice and ACCHO teams are targeting the highest need patient cohort. Quarantining Practice Nurse time to assist with the preparation of GPCCMPs, reviews and health assessments ensures that KHPMP eligible patients are receiving proactive chronic disease prevention and mangament, with structured follow up.
